Application Process & Required Documents

- ADN Multi Criteria Screening Worksheet: Unofficial document for student/counselor reference use only. Do not submit this form with your ADN Application.
Apply to the traditional ADN program or the LVN to RN Advanced Placement program in 2 easy steps!
Step 1:
- All students must complete the ADN Multi Criteria Evaluation Request Form. (The evaluation
request form will only be processed during the following evaluation period: August
5th, 2024, 8:00 a.m. through September 20th, 2024, by 4:00 p.m.
You will be prompted to log into your Chaffey College Portal and routed to the form once you click on the form link below.
ADN Multi Criteria Evaluation Request Form - Once you receive your official ADN Multi Criteria Evaluation Form returned from the
Evaluation Office via email, use the respective supplemental application documents
and application links to apply to the program of your choice.
Step 2:
Prepare supplemental application documents: Please use the following downloadable supplemental documents for consideration of extra points to be submitted with your ADN application (if applicable). * LVN to RN Advanced Placement applicants do not need to complete these forms for their applications.
- Recent work/volunteer experience verification form
- Demonstrated proficiency in a language other than English verification form
- Life experience/special circumstances area
